Introduction Leukodystrophies (LDs) describe a class of various diseases of the white matter caused by genetic defects leading to a disturbed myelination. In our study we defined histologic characteristics of various Lds for better distinguishment and differentialdiagnostic analysis. Methods We collected and analyzed tissue samples from three patients suffering from metachromatic leukodystrophy (MLD), seven patients suffering from x-chromosomal adrenoleukodystrophy (X-ALD), three patients suffering from Vanishing White Matter Disease (VWMD) and two patients suffering from Morbus Alexander. The samples were collected at the institute of neuropathology at the University Hospital Göttingen. We quantified the extent of demyelination of the gray and white matter, the axonal damage, the inflammatory infiltration, the level of the activity of macrophages, the reactive gliosis and the intensity of the damage of oligodendrocytes. We used twelve control samples of patients with the same age and no abnormalities at the neuropathologic autopsy in order to compare our quantifications. Results Extensive, a little bit patchy demyelination of the white matter wihout the involvement of U-fibres was observed in cases of MLD, VMD and X-ALD. We only observed a cortical demyelination in VWMD. More typical characteristics were a high degree of infiltrations of macrophages with macrophages having incorporated cytoplasmic material. Macrophages in MLD patients seemed to be large, tightly packed, with partly containing bluish cytoplasmic vacuoles. Interestingly we also saw apoptosis of macrophages in MLD. The characteristics of patients with X-ALD were colorless, mainly centrally located clefts within the cell body of macrophages and astrocytes as well as a lymphocytic inflammatory reaction. The quantifications of the extent of the axonal damage were quite heterogenous. We detected a distinct gliosis in all variants of LDs collected in this study. Dystrophic astrocytes with white deposits of IgG and fibrinogen in their cell body and processes were found in cases of VWMD. Plenty of perivascular, subependymale Rosenthalfibres were detected in M. Alexander. As for MLD, X-ALD and VWMD a marked loss of oligodendrocytes was observed correlating partly with the degree of demyelination. Findings of apoptosis of oligodendrocytes were only noted occasionally. In addition to that oligodendrocytes in patients suffering from VWMD showed a huge, compact, bluish eosinophile cell body. Inflammatory infiltrates were only found in cases of X-ALD and M. Alexander. Discussion The results of our study help distinguishing the various LDs and are beneficial in the process of differentialdiagnosis with diseases like multiple sclerosis, acute disseminated encephalomyelitis, neuromyelitis optica, infectious diseases like multifocal leukoencephalopathy and mitochondrial disease like Morbus Leigh.

Leukodystrophie sind eine Gruppe von unterschiedlichen Erkrankungen der weißen Substanz mit genetischer Ursache, die eine gestörte Myelinisierung zur Folge haben. Mit der Arbeit haben wir histologische Merkmale verschiedener LDs herausgearbeitet, um die LDs untereinander und von ihren Differentialdiagnosen abzugrenzen. Methoden: Wir haben Gewebeproben von insgesamt drei Patienten mit einer metachromatischen Leukodystrophie (MLD), sieben Patienten mit einer X-chromosomalen Adrenoleukodystrophie (X-ALD), drei Patienten mit einer Vanishing White Matter Disease (VWMD) und zwei Patienten mit einem Morbus Alexander aus dem Institut der Neuropathologie der Universitätsmedizin Göttingen histologisch und immunhistochemisch untersucht. Hierbei wurden das Ausmaß der Entmarkung in der grauen und weißen Substanz, die axonale Schädigung, entzündliche Infiltration, Makrophagenabräumreaktion, die reaktive Gliose und das Ausmaß der Oligodendrozytenschädigung beurteilt und quantifiziert. Als Vergleich für die Quantifizierungen dienten Gewebeproben von insgesamt 12 Kontrollfällen gleichaltrigen, deren neuropathologischer Autopsiebefund keine wesentlichen pathologischen Veränderungen ergab. Ergebnisse: Flächige, teils fleckige Entmarkung in der weißen Substanz des Großhirns ohne Mitbeteiligung der U-Fasern war bei den MLD-, VWMD- und X-ALD-Fällen feststellbar. Eine kortikale Entmarkung konnte nur bei der VWMD nachgewiesen werden. Weitere typische Kennzeichen sind zahlreiche Makrophageninfiltrate und Makrophagen mit inkorporiertem zytoplasmatischen Material. Die Makrophagen bei der MLD erscheinen groß, dicht bepackt, teils mit bläulichen zytoplasmatischen Vakuolen. Interessanterweise finden sich bei der MLD auch Makrophagenapoptosen. Charakteristisch fanden sich bei der X-ALD farblose, oftmals zentral liegende Spalten im Zellleib der Makrophagen und Astrozyten sowie eine lymphozytäre Entzündungsreaktion. Das Ausmaß der axonalen Schädigung ist äußerst variabel zwischen den verschiedenen Leukodystrophien. Eine deutliche Gliose ist bei allen Leukodystrophie-Formen erkennbar. Bei der VWMD erscheinen die Astrozyten dystroph und weisen Ablagerungen von IgG und Fibrinogen an ihrem Zellleib und Fortsätzen auf. Beim M. Alexander zeigten sich perivaskulär, subependymal massenhafte Rosenthalfasern. Bei der MLD, X-ALD und VWMD ist ein deutlicher Verlust an Oligodendrozyten, teils korrelierend zum Entmarkungsgrad, ersichtlich. Vereinzelt sind Oligodendrozytenapoptosen nachweisbar. Bei der VWMD weisen die Oligodendrozyten zudem einen deutlich vergrößerten, kompakten und blass eosinophil erscheinenden Zellleib auf. Entzündliche Infiltrate finden sich nur bei der X-ALD und dem M. Alexander. Interpretation: Die herausgearbeiteten histologischen Charakteristika helfen zum einen die verschiedenen LDs untereinander zu unterscheiden und zum anderen von deren Differentialdiagnosen abzugrenzen, wobei zu diesen entzündliche Entmarkungskrankheiten, wie Multiple Sklerose, akute disseminierte Enzephalomyelitis, Neuromyelitis Optica; infektiöse Erkrankungen, wie progressive multifokale Leukenzephalopathie; Erkrankungen des ZNS durch Elektrolytschwankungen, wie zentrale pontine Myelinolyse, und mitochondriale Erkrankungen, wie Morbus Leigh, gehören.
