dc.description.abstractgerDie laterale Patellaluxation gehört zu den häufigen Verletzungen des Kniegelenkes und betrifft vor allem junge Menschen in den ersten zwei bis drei Lebensdekaden. Viele prädisponierende Faktoren für das Auftreten einer Patellaluxation sind bereits bekannt, trotzdem bleibt das Erkrankungsbild Gegenstand der aktuellen Forschung um die Bedeutung der knöchernen, knorpeligen, ligamentären und muskulären Begebenheiten besser zu verstehen und die Therapiemöglichkeiten stetig zu optimieren. Als einer der Hauptrisikofaktoren für das Auftreten einer Patellaluxation hat sich die Trochleadysplasie als knöcherne Veränderung des ventralen distalen Femurs herausgestellt. Ergebnisse einiger Studien legen nahe, dass die Geometrie des Tibiaplateaus entscheidenden Einfluss auf die Biomechanik des femorotibialen Gelenks hat. Die Auswirkungen einer veränderten Morphologie des Femorotibialgelenks auf das Patellofemoralgelenk, und damit auf die Luxationstendenz der Patella, wurden in dieser Studie untersucht. Hierfür wurden 83 Kniegelenke von Kontrollpatienten und 107 Kniegelenke von Patienten mit lateraler Patellainstabilität MR-tomographisch untersucht. Anhand der MR-Bilder wurden der laterale und mediale tibiale Slope gemessen sowie der anatomische distale laterale Femurwinkel und der mechanische proximale mediale Tibiawinkel bestimmt. Außerdem erfolgte die Einteilung aller untersuchten Kniegelenke nach dem Grad der Trochleadysplasie anhand der Kriterien von Dejour (normal oder Grad A-D). Alle Parameter wurden durch eine lineare Regression mit der Trochleadysplasie als unabhängige Variable individuell beurteilt. Es zeigte sich, dass der Schweregrad der Trochleadysplasie signifikant assoziiert ist mit einer asymmetrischen Inklination des Tibiaplateaus. Während der mediale Slope identische Werte bei den Studienpatienten und dem Kontrollkollektiv zeigte, wird die Inklination des lateralen Slopes mit zunehmendem Grad der Trochleadysplasie signifikant flacher. Durch den gleichbleibenden medialen Slope entsteht eine kontinuierlich zunehmende intraindividuelle Slopedifferenz. Hieraus folgt eine verstärkte femorale Innenrotation in 20° und 90° Knieflexion bei Patienten mit Trochleadysplasie. Die Ergebnisse der Studie weisen auf einen Zusammenhang zwischen der Morphologie des Tibiaplateaus und dem Ausmaß an femoraler Rotation bei Patienten mit lateraler Patellainstabilität und zugrunde liegender Trochleadysplasie hin. Die intraindividuelle Slopedifferenz verursacht eine zunehmende femorale Innenrotation während der Kniegelenksflexion und könnte dadurch den Effekt der femoralen Anteversion verstärken.de
dc.description.abstractengLateral patellar instability is a common injury to the knee joint in adolescents and young adults. In recent years many predisposing factors have been identified and treatment options are continuously evolving for the underlying anatomic conditions. Several studies have shown that the geometry of the tibial plateau influences the biomechanics of the tibiofemoral joint. However, whether patients with lateral patellar instability also have morphologic alterations at the tibial plateau with potential impact on tibiofemoral and patellofemoral biomechanics has not been evaluated yet. Therefore, the purpose of this study was to evaluate the inclination of the medial and lateral tibial slope in patients with lateral patellar instability and to investigate whether tibial slope characteristics might influence the biomechanics of the tibiofemoral and the patellofemoral joint. For the purpose of this study we measured the medial and lateral tibial slope using magnetic resonance imaging (MRI) in 107 patients with lateral patellar instability and in 83 controls. In addition, trochlear dysplasia was assessed in all participants and graded according to Dejour’s criteria (normal or type A-D). The medial tibial slope did not differ significantly between patients and controls, whereas the values of the lateral tibial slope decreased significantly with increasing severity of trochlear dysplasia. Patients with a higher degree of trochlear dysplasia showed an asymmetric inclination of their tibial plateau due to normal inclination of the medial slope but decreased inclination of the lateral slope. In order to assess the impact of slope asymmetry on the patellofemoral joint, the internal femoral rotation in each subject was calculated by means of radian measure in 20° and 90° knee flexion. Results indicated that an increased slope asymmetry enhances the internal femoral rotation during knee joint flexion. In summary, the results of this study indicate an association between the morphology of the tibial plateau and the severity of trochlear dysplasia. Thereby, increasing tibial slope asymmetry might enhance the internal femoral rotation during knee joint flexion in patients with severe trochlear dysplasia.de
