We evaluate in a prospective and randomized study the use of the Silent Operating Theatre Optimization System in radical laparoscopic robot-assisted prostatovesiculectomy at our clinic. The present study consisted of two study groups operating with and without SOTOS.  In both groups, there were no significant differences in length of operation, intraoperative difficulty, radicality, complications, postoperative infections and lying time. In the SOTOS group, a trend towards shorter surgical times and optimal local tumor control during surgery was observed. An effect on the heart rates of the participants and thus on the feeling of stress could not be shown.  Measurements were carried out in the operating room at specific times of operation in terms of noise levels and their impact on the participants. Sound pressure levels and sound pressures were significantly lower compared to the control group in the SOTOS group. This is reflected over the entire duration of the operation. We were able to show that the use of SOTOS in radical laparoscopic robot-assisted prostatovesiculectomies allows a significant noise reduction in the operating room.
