dc.description.abstractgerDie diabetische Nephropathie, eine mikrovaskuläre Komplikation des Diabetes mellitus, stellt eine bedeutsame Ursache der chronischen Nierenerkrankung dar. Sie ist gekennzeichnet durch ein erhöhtes kardiovaskuläres Risiko und einer Progredienz bis zur terminalen Niereninsuffizienz. E-Cadherin ist ein kalziumabhängiges Zelladhäsionsprotein epithelialer Zellen, wobei der Cadherin-Catenin-Komplex der Hauptmediator der Zelladhäsion ist und wesentlich für die epitheliale Integrität und den epithelialen Phänotyp ist. E-Cadherin ist zudem an verschiedenen Signalkaskaden beteiligt. Der Verlust des E-Cadherins ist bei zahlreichen malignen Erkrankungen beschrieben. Ein wichtiger Mechanismus ist die proteolytische Spaltung des E-Cadherins an der Zelloberfläche, wodurch ein 80 kDa schweres extrazelluläres Fragment, das lösliche E-Cadherin (sE-cad), entsteht. Aktuelle Studien zeigen, dass sE-cad im Urin ein potenzieller Frühmarker einer diabetischen Nephropathie ist und die Urinkonzentration des sE-cads mit der Ausprägung der diabetischen Nephropathie korreliert. Die Herkunft des Urin-sE-cads wurde im Rahmen dieser Arbeit untersucht. Die Behandlung distaler Tubulusepithelzellen (MDCK-Zellen), die E-Cadherin exprimieren, mit TGF-ß1 führte durch die Aktivierung des TGF-ß1-/Smad-Signalwegs zur Freisetzung von sE-cad. Die Behandlung mit TGF-ß1 in Kombination mit dem Matrix-Metalloproteinasen-Inhibitor GM6001 führte zu einer verminderten sE-cad-Freisetzung. Dies deutet auf renale Tubulusepithelzellen als möglichen Ursprung des sE-cads im Urin bei diabetischer Nephropathie hin. Eine Wirkung des sE-cads auf renale Zellen ist bisher nicht erforscht. Um eventuelle protektive oder schädigende Effekte des sE-cads zu untersuchen, wurden im Rahmen dieser Arbeit proximale Tubulusepithelzellen (HK-2-Zellen), distale Tubulusepithelzellen (MDCK-Zellen) und renale Fibroblasten (TK173-Zellen) mit unterschiedlichen sE-cad-Konzentrationen behandelt. Im MTT- und Apoptose-Assay zeigte sich nach sE-cad-Behandlung eine verminderte Zellviabilität der Tubulusepithelzellen. Eine Zunahme der Zellviabilität und -proliferation war in renalen Fibroblasten erkennbar. Im Western Blot, der Immunfluoreszenzfärbung und der qPCR wurden Effekte auf die Expression von Fibrosemarkern und auf den Signalweg um E-Cadherin und ß-Catenin analysiert. Es ergaben sich Hinweise auf eine potenzielle protektive Wirkung des sE-cads auf distale Tubulusepithelzellen: die Expression der Fibrosemarker und des ß-Catenins nahm ab, die Expression des E-Cadherins zu. In renalen Fibroblasten nahm hingegen die Expression der Fibrosemarker zu. Diese Ergebnisse waren statistisch nicht signifikant. Interessant ist die konträre Wirkung des sE-cads auf renale Fibroblasten und distale Tubulusepithelzellen, welche auf eine zelltypspezifische Wirkung des sE-cads hinweist. Es bedarf weiterer Untersuchungen, um die komplexe Rolle des sE-cads in der diabetischen Nephropathie zu verdeutlichen und weitere Erkenntnisse in der Pathogenese und Therapie der diabetischen Nephropathie zu gewinnen.de
dc.description.abstractengDiabetic nephropathy is a microvascular complication of diabetes mellitus and an important cause of chronic kidney disease. It is characterized by an increased cardiovascular risk and progression to terminal renal failure. E-cadherin is a calcium dependent cell adhesion protein of epithelial cells, where the complex E-cadherin/catenin is the main mediator of cell adhesion. This complex is essential for the epithelial phenotype and in maintaining epithelial integrity. E-cadherin is also involved in several signal transduction pathways. The loss of E-cadherin is described in numerous malignant diseases. An important mechanism is the proteolytic cleavage of E-cadherin at the cell surface resulting in the release of an 80 kDa large extracellular fragment, the soluble E-cadherin (sE-cad). Recent studies identified sE-cad as an early marker for renal injury in the urine of diabetes patients and demonstrated that the amount of sE-cad in urine correlated with the degree of kidney injury. The origin of urine sE-cad was investigated in this work. Treatment of distal tubule cells (MDCK cells) expressing E-cadherin with TGF-ß1 and activation of TGF-ß1/Smad signaling resulted in the release of sE-cad. Moreover, TGF-ß1 treatment combined with the matrix metalloproteinase inhibitor GM6001 led to a reduced release of sE-cad suggesting renal epithelial cells as potential origin of sE-cad in urine of diabetic nephropathy patients. Whether the urine sE-cad has a protective or harmful effect on renal cells is not known. To investigate the effect of sE-cad on renal cells, proximal tubule cells (HK-2 cells), distal tubule cells (MDCK cells) and renal fibroblasts (TK173 cells) were treated with different sE-cad concentrations. MTT- and apoptosis-assays revealed a decrease in cell viability of tubule cells upon sE-cad treatment. An increase in cell viability and proliferation was detected in renal fibroblasts. To investigate both the expression of fibrosis markers and the effects on the E-cadherin/catenin pathway, western blot, immunofluorescence staining and qPCR were used. The resulting data indicated a potential protective effect of sE-cad on distal tubule cells: the expression of the fibrosis markers and ß-catenin decreased, the expression of the E-cadherin increased. In contrast, the expression of the fibrosis markers increased in renal fibroblasts. These results were not significant. The contrary effects of sE-cad on renal fibroblasts and distal tubule cells are very interesting and reveal cell type specific action of sE-cad. Further investigations are still required to clarify the complex role of sE-cad in diabetic nephropathy and to gain further knowledge in the pathogenesis and therapy of diabetic nephropathy.de
