Acute cerebral ischaemia is regarded as the second leading cause of death worldwide and the main cause of longstanding or permanent disability. Atrial fibrillation-associated cerebral ischaemia is related to a high risk of recurrence compared to that of other aetiologies. However, the risk of thromboembolism can be minimised by adequate anticoagulation, which is why initiating anticoagulation can be considered to be a highly effective therapy. Thus, the detection of (paroxysmal) atrial fibrillation, although associated with difficulties due to the often silent and short episodes, is the basis for adequate primary and secondary prevention of cardioembolic events. In this study, the long-term recurrence rate of (paroxysmal) atrial fibrillation was investigated in 281 patients with acute cerebral ischaemia over a period of approximately three years, and the overall morbidity and mortality were compared between patients with acute cerebral ischaemia with and without confirmed atrial fibrillation. For this purpose, patients were followed up approximately three years after TIA or ischaemic stroke and their medical history was collected since the index event. The data obtained by electrocardiography not only prove the persistence of atrial fibrillation in patients with atrial fibrillation first diagnosed in the context of acute cerebral ischaemia, but also show a clear tendency towards progression over the time. From this it can be deduced that atrial fibrillation detected for the first time around a stroke is also relevant for the long-term course and should not only be regarded in the context of or as a consequence of the acute event and that permanent oral anticoagulation therefore appears to be highly recommended in these patients. In a comparison of 24 atrial fibrillation patients (index patients) with 25 age- and sex-matched control patients, a higher overall mortality, but not a higher cardiovascular mortality of the atrial fibrillation patients could be identified. With few numbers of cases, there tended to be more hospitalisations among the index patients, while the recurrence rates for cerebral ischaemia were comparable. However, a significantly higher percentage of index patients continued to have atrial fibrillation after three years compared to control patients, again demonstrating the relevance of atrial fibrillation episodes detected after the initial cerebral ischaemia for the persistence of the disease.

Die akute zerebrale Ischämie gilt als zweithäufigste Todesursache weltweit und als Hauptursache für eine lang bestehende bzw. bleibende Invalidität. Vorhofflimmer-assoziierte zerebrale Ischämien sind im Vergleich zu denen anderer Ätiologie mit einem hohen Rezidivrisiko verbunden. Das Thromboembolierisiko lässt sich jedoch mittels einer adäquaten Antikoagulation auf ein Minimum reduzieren, weshalb die Einleitung einer solchen als hocheffektive Therapie angesehen werden kann. Demnach stellt die Detektion von (paroxysmalem) Vorhofflimmern, wenn auch durch die häufig stummen und kurzen Episoden mit Schwierigkeiten verbunden, die Grundlage für eine adäquate Primär- und Sekundärprävention kardioembolischer Ereignisse dar. In dieser Arbeit wurde die Langzeitrezidivrate von (paroxysmalem) Vorhofflimmern bei 281 Patienten mit akuter zerebraler Ischämie über einen Zeitraum von circa drei Jahren untersucht sowie die Gesamtmorbidität und -mortalität zwischen Patienten mit akuter zerebraler Ischämie mit und ohne nachgewiesenes Vorhofflimmern verglichen. Hierfür wurden die Patienten circa drei Jahre nach stattgehabter TIA bzw. ischämischem Schlaganfall nachuntersucht und deren Krankheitsanamnese seit dem Index-Ereignis erhoben. Die elektrokardiographisch gewonnenen Daten belegen nicht nur die Persistenz von Vorhofflimmern bei Patienten mit im Rahmen einer akuten zerebralen Ischämie erstdiagnostiziertem Vorhofflimmern, sondern zeigen auch eine deutliche Tendenz zur zeitlichen Progredienz im Verlauf auf. Hieraus lässt sich ableiten, dass um einen Schlaganfall erstmals detektiertes Vorhofflimmern auch für den Langzeitverlauf relevant ist und nicht nur im Rahmen bzw. als Folge des Akutgeschehens gesehen werden darf und dass bei diesen Patienten folglich eine dauerhafte orale Antikoagulation als sehr empfehlenswert erscheint. Im Vergleich von 24 Vorhofflimmerpatienten (Index-Patienten) mit 25 alters- und geschlechtsgematchten Kontrollpersonen konnte eine höhere Gesamtmortalität, nicht aber eine höhere kardiovaskuläre Mortalität der Vorhofflimmerpatienten ausgemacht werden. Bei kleinen Fallzahlen zeigten sich tendenziell mehr Hospitalisierungen bei den Index-Patienten, während die Rezidivraten für zerebrale Ischämien vergleichbar waren. Allerdings konnte bei einem signifikant höheren Prozentsatz der Index-Patienten im Vergleich zu den Kontrollpatienten nach drei Jahren (weiter) Vorhofflimmern nachgewiesen werden, was nochmals die Relevanz der nach dem initialen Schlaganfall detektierten Vorhofflimmer-Episoden für die Persistenz der Erkrankung belegt.
