dc.description.abstractgerEine komplette Röntgendbefundung stellt einen Grundpfeiler der kieferorthopädischen Diagnostik dar. So ist neben der Analyse bzgl. des Hauptfokus der Bildgebung auch eine umfassende Diagnostik aller Nebenbefunde – abseits der dentalen bzw. dentoalveolären Regionen – obligat. Von einer Zunahme der Nebenbefunde in der Röntgendiagnosttik ist vor dem Hintergrund des stetigen Zulaufes erwachsener Patienten in den kieferorthopädischen Praxen auszugehen.  Daraus ergibt sich die klinisch relevante Frage, ob ein Alterseffekt bzgl. der Prävalenz, der Lokalisation und des Schweregrades von Nebenbefunden in der kieferorthopädischen Röntgendiagnostik existiert?  Die klinisch, primär-retrospekive Studie untersuchte pathologischer Nebenbefunde von insgesamt 600 OPG- und FRS-Aufnahmen anhand zweier Gruppen kieferorthopädischer Patienten (Gruppe U18: 6-17 Jahre; Gruppe Ü18: 18-72 Jahre). Prävalenz, Lokalisation und Schweregrad der Befunde wurden in einem konzipierten Klassifikationsbogen aufgezeichnet. Die Befundung sah ein systematisches Vorgehen entlang der neun Lokalisationen (Mandibula, Maxilla, Dentition, Nasennebenhöhlen, Kiefergelenk, Schädelbasis, Orbita, Halswirbelsäule, Weichgewebe) durch drei erfahrene Prüfärzte vor.  Insgesamt konnten 1.458 Nebenbefunde bei allen Patienten nachgeweisen werden, wobei 66 % der Nebenbefunde abseits der Dentition auftraten. Es zeigte sich ein signifikanter Alterseffekt (p < 0,001) bzgl. der Prävalenz der Nebenbefunde (Gruppe Ü18 – erwachsene Patienten 1.026 Befunde – OPG: 566 / FRS 460 vs. Gruppe U18 – Kinder / jugendliche Patienten 432 Befunde – OPG: 221 / FRS 211). Bzgl. der Lokalisation waren die Nebenbe- funde bei Erwachsenen am häufigsten in den Regionen Dentition, NNH und Mandibula zu finden. Zudem ergaben sich in der Analyse der FRS-Aufnahmen bei Erwachsenen im Bereich der Schädelbasis und HWS signifikant mehr Nebenbefunde (p < 0,01), wohingegen bei Kindern/Jugendlichen in der Region der Weichgewebe mehr Nebenbefunde zu finden waren.  Die Kategorisierung nach Schweregrad der Nebenbefunde zeigte, dass 33 % der Nebenbefunde eine weiterführende Diagnostik und gegebenfalls Therapie durch weitere Fachdisziplinen erforderlich machten.  Die kieferorthopädische Röntgendiagnostik bietet ein großes Befundungsareal, indem Nebenbefunde abseits der Dentition mit hoher Prävalenz auftreten können. Insbesondere bei Erwachsenen ist von einer Vielzahl von Nebenbefunden auch außerhalb der dentalen/ alveolären Region auf kieferorthopädischen Röntgenbildern auszugehen. Daher ist ein standardisiertes Vorgehen während der Befundung notwendig, um das Übersehen von Nebenbefunden mit klinischer Relevanz zu minimieren.de
dc.description.abstractengBackground: Panoramic radiographs and lateral cephalogram open a great diagnostic yield to detect occult pathologic events outside the dental area. Frequent pathological abnormalities were depicted in several clinicoradiological studies. Does an aging of orthodontic population effect an enhancement of secondary findings, that can be read in orthodontic radiographs? The aim of the study is to proof the age effect regarding to the prevalence of incidental in orthodontic radiographs outside the dental region. Methods: The retrospectiv study included 600 pretreatment orthodontic radiographs of 150 infantile and 150 adult patients with ages ranging from 6 to 72 years. The mean age was 19.46 years, with a standard deviation of 10.71 years. All scans were evaluated by three experienced dentists and each incidental finding was further classified along nine locations and by its pathological character. Results: These studies evidenced a positiv age effect of incidental findings in different extradental locations within the appraisal of orthodontic radiographs. A total of 1458 incidental findings were reported. On average 2.6 incidental findings were observed in each panoramic radiograph. The lateral cephalogram performed on average 2.2 pathological abnormalities. Apart of the dentition (n = 497), the most common affected regions were the para- nasal sinus (n = 266), mandibula (n = 174), cranial base (n = 160), soft-tissue (n = 130) and maxilla (n = 124). Adult radiographs embodied a statistically significant higher amount of incidental findings in the following localizations: dentition, maxilla, mandibula, paranasal sinus and temporomandibular joint (p < 0,001). In the lateral cephalogram adults tended to show pathological events in the regions cranial base and cervical spine more frequently (p < 0,05). Thirty-three percent of all findings needed follow up for further diagnostics, as the circumstances required medical attendance before orthodontic treatment. Conclusions: This study confirms the higher prevalence of incidental findings in the sample of adults. Since incidental findings are common and can be easily detected in orthodontic radiographs, orthodontics should be aware of the multiplicity of secondary findings outside the dental area. The current studies proposed a standardized approach in diagnostic radiology before orthodontic treatment. Common incidental findings are discussed and their clinical relevance is explained.de
