dc.description.abstractger | Die intensivierte konventionelle Therapie wurde ursprünglich zur Therapie des Typ-1-Diabetes entwickelt und gilt hierfür als Standardtherapie, wird aber auch bei Patienten mit Typ-2-Diabetes beim Sekundärversagen der oralen Antidiabetika durchgeführt.
Die vorliegende prospektive Studie hatte zum Ziel, die Unterschiede in der Durchführung, den Charakteristika und im Besonderen der Blutzuckereinstellungsqualität der intensivierten konventionellen Insulintherapie bei Patienten mit Typ-1-Diabetes im Vergleich zu Patienten mit Typ-2-Diabetes herauszuarbeiten.
Hauptzielkriterium war der Variationskoeffizient des über 14 Tage gemessenen Nüchternblutzuckers. Es wurden erratischere Blutzuckerschwankungen bei den Patienten mit Typ-1-Diabetes erwartet.
An der Studie beteiligten sich je 28 Patienten mit Typ-1- bzw. Typ-2-Diabetes. Sie dokumentierten über einen 14-tägigen Zeitraum so genau wie möglich alle Aspekte ihrer intensivierten konventionellen Insulintherapie.
Als Schwerpunkt konnte die vorliegende Studie hervorbringen, dass die Patienten mit Typ-1-Diabetes insgesamt im Tagesprofil mit ihren Blutzuckerwerten signifikant höher (p = 0,034) lagen als die Patienten mit Typ-2-Diabetes. Außerdem war zu allen präprandialen Zeitpunkten und vor dem Zubettgehen der Variationskoeffizient für die Blutzucker von Tag zu Tag bei Patienten mit Typ-1-Diabetes signifikant höher, stark signifikant am Morgen (p-Wert < 0,0001).
Die neu gewonnen Ergebnisse könnten helfen, die Behandlung der Patienten zu optimieren. So ist die Zuordnung eines Patienten zur Gruppe derer mit Diabetes Typ 1 oder 2 nicht immer eindeutig. Aufgrund der Ergebnisse erscheint es nun möglich, sich die Variabilität des Blutzuckers als Unterscheidungsmerkmal zunutze zu machen, so dass sie evtl. Anlass zur Reklassifizierung geben könnte. Fehlzuordnungen könnten so aufgedeckt und die Therapie des Patienten optimiert werden. | de |
dc.description.abstracteng | The intensive insulin therapy was originally developed as treatment for type 1 diabetics and is still mainstay of treatment today. Additionally, it is used as secondary therapy for patients with type 2 diabetes, in whom oral hypoglycaemic treatment has failed.
Aim of this prospective study was to assess the differences in features, execution, and especially the quality of blood glucose control in the treatment of patients with type 1 diabetes in comparison to type 2 diabetics receiving intensive insulin therapy.
Primary endpoint of the study was the coefficient of variation of the fasting blood glucose measured at different points over 14 days. We expected a more unpredictable variation of blood glucose in patients with type 1 diabetes.
28 patients with type 1 or 2 diabetes were included in the study. Patients recorded all aspects of their intensified insulin therapy over 14 days.
This prospective study showed that patients with type 1 diabetes had a significantly higher blood glucose level over all measurements per day (p= 0,034) than patients with type 2 diabetes. Additionally, the coefficient of variation in type 1 diabetics was significantly higher at all pre-prandial measurements and especially in the mornings (p< 0,0001).
We believe that these results could help improve the treatment of patients with diabetes. It is not always clear, whether a patient suffers from type 1 or type 2 diabetes and this study shows, that the variability of blood glucose levels could help to distinguish between the two types and hence lead to a change of diagnosis and improved treatment of these patients. | de |
