Aims: Carvedilol is a beta blocker of the 3rd generation. Still little known is the relation between dependence on the CYP2D6-polymorphism on carvedilol pharmakokinetics and carvedilol pharmacodynamics. Furthermore the enzymes CPY2C9 as well as CYP2C19 shall play a role in the metabolism of carvedilol. Therefore we examined the effects of CYP2D6-, CYP2C9- and CYP2C19-polymorphisms on pharmacokinetics and effects of carvedilol. Methods: After carvedilol 25 mg was administered to 110 healthy volunteers, on the one hand concentrations of carvedilol were determined and on the other hand effects on resting and exercise-induced heart rate and blood pressure were examined. Finally we analysed our results by using population pharmacocinetic, pharmacodynamic and pharmacogenetic modeling. Results: Significant differences could be seen in carvedilol pharmacokinetics in relation to the CYP2D6-polymorphism. No significant differences could be seen in carvedilol pharmacokinetics in relation to the CYP2C9- as well as CYP2C19-polymorphisms. Finally the CYP2D6-, CYP2C9- and CYP2C19- genotype had no effect on heart rate, blood pressure or adverse effects. Conclusion: Significant CYP2D6 allele-specific differences in carvedilol pharmacokinetics are seen, but there are no pharmacodynamic variation of carvedilol in releation to the CYP2D6-polymorphism.

3. Generation. Noch wenig bekannt ist der Zusammenhang zwischen der
Abhängigkeit vom CYP2D6-Polymorphismus auf die
Carvedilol-Pharmakokinetik und die Carvedilol-Pharmakodynamik.
Weiterhin sollen die polymorphen Enzyme CPY2C9 sowie CYP2C19 im
Carvedilolmetabolismus eine Rolle spielen. Daher haben wir die
Auswirkungen von CYP2D6-, CYP2C9- und CYP2C19-Polymorphismen auf
Pharmakokinetik und Wirkung von Carvedilol untersucht. Methoden:
Nachdem 110 gesunden Probanden 25 mg Carvedilol appliziert wurde,
wurde einerseits die Konzentration von Carvedilol bestimmt und
andererseits die Wirkung auf die Herzfrequenz und den Blutdruck in
Ruhe und unter Belastung untersucht. Schließlich wurden
pharmakokinetische, pharmakodynamische und pharmakogenetische
Analysen durchgeführt. Ergebnisse: Es konnten in Relation zum
CYP2D6-Polymorphismus signifikante Unterschiede in der
Pharmakokinetik gesehen werden. In Abhängigkeit vom CYP2C9- sowie
CYP2C19-Polymorphismus konnten keine wesentlich signifikanten
Unterschiede in der Carvedilol-Pharmakokinetik gesehen werden.
Ebenfalls konnten wir in Relation zu den 3 polymorphen Enzymen
keine signifikanten Unterschiede in der Carvedilol-Wirkung auf
Herzfrequenz, Blutdruck oder Nebenwirkungen erkennen.
Schlussfolgerung: Zwar bestehen in Abhängigkeit zum
CYP2D6-Polymorphismus in der Carvedilol-Pharmakokinetik
signifikante Unterschiede, jedoch sind diese klinisch nicht
bedeutend, da in Relation zum CYP2D6-Polymorphismus keine
Unterschiede in der Carvedilol-Pharmakodynamik beobachtet werden
konnten.
