In this present study 82 patients with previously untreated squamous cell carcinoma of the tongue base (T1-4, N0-2, M0) underwent transoral laser surgery with curative intent between 1986 and 2011. 5-year overall survival and recurrence-free survival was 59% and 69%, respectively. In 11 (13%) patients local recurrence occurred. Primary transoral laser microsurgery in combination with adjuvant radio(chemo)therapy is an efficient treatment for early stages and advanced tongue base carcinoma that achieves good local control and overall-survival rates. Compared with primary high-dose radio(chemo)therapy it offers better oncologic results combined with significantly lower functional deficits and fewer side-effects. Compared with conventional surgery, transoral laser microsurgery shows less inherent complications. Hence, multimodal treatment can be advantageous and should be considered for all advanced stages to achieve maximum disease control.
