dc.description.abstract | Epidemiologische und morphologische
Unterschiede zwischen proximalen und distalen kolorektalen
Karzinomen basieren teilweise auf der unterschiedlichen
Lokalisation. Beim kolorektalen Karzinom (KRK) wurden zwei
pathogenetisch unterschiedliche Entstehungsmuster beschrieben,
welche mit der Lokalisation (proximal versus distal) assoziiert
sind. Das Ziel dieser Arbeit war es mögliche Unterschiede in
chromosomalen Mustern und die daraus resultierende Prognose
zwischen 26 proximalen und 49 distalen KRK mit einem
pT3-Tumorstadium mittels komparativer genomischer Hybridisierung
(CGH) zu ermitteln. Proximal lokalisierte KRK zeigen häufiger eine
Mikrosatelliteninstabilität und signifikant seltener chromosomale
Brüche und Instabilität als distal lokalisierte KRK; v.a. seltener
folgende Veränderungen: -18q (p=0.001), +20q (p=0.006), -14q
(p=0.04) und -15q (p=0.006). Hinzu kommt, dass das proximale KRK
häufiger normale Karyotypen im Genom aufzeigt als das distale KRK.
Obwohl distale und proximale KRK ähnliche chromosomale Muster
aufweisen, fehlen trotzdem Marker, die eine genaue Differenzierung
zwischen diesen beiden Lokalisationen erlauben. Darüber hinaus gab
es sogar ähnliche Assoziationen zwischen speziellen Veränderungen
und der Prognose oder dem Überlben, wie z.B. der Zusammenhang
zwischen dermVerlust an 4q und einem positiven Lymphknotenstatus.
Zusammenfassend lenken die Beobachtungen dieser Arbeit die
Aufmerksamkeit beim KRK auf eine lokalisationsspezifische
Evaluierung molekularer Marker, die eventuell das biologische
Verhalten und somit die Prognose in proximalen und distalen KRK
prognostizieren können. Aus diesem Grund sind größere prospektive
Studien denkbar, die die seitenspezifische Wertigkeit von
zytogenetischen und molekularen Markern untersuchen können. | de |

dc.description.abstracteng | Epidemiologic and morphologic differences
between proximal colonic carcinomas and rectal carcinomas, point to
different etiologic backgrounds and to the existence of at least
two different categories of colorectal cancer broadly based on site
of origin. The aim of the present study was to investigate the
possible differences in chromosomal profiles between 26 pT3 colonic
carcinomas proximal to the splenic flexure and 49 pT3 rectal
carcinomas using comparative genomic hybridization. Proximal
colonic carcinomas revealed higher rates of microsatellite
instability and overall significantly fewer chromosomal breaks and
chromosomal imbalances than rectal carcinomas, including fewer -18q
(P = 0.001), +20q (P = 0.006), -14q (P = 0.04), and -15q (P =
0.006). Also, colonic carcinomas tended to have higher rates of
normal karyotypes than rectal carcinomas. However, colonic
carcinomas and rectal carcinomas shared similar chromosomal
patterns and there was no universal chromosomal marker that made
the distinction between all 26 colonic carcinomas and 49 rectal
carcinomas. Moreover, there were similar associations for specific
imbalances with prognostic parameters or survival within the
subgroups of colonic and rectal tumours, including -4q with
positive pN-stage. In conclusion, the present findings lend support
to the concept that proximal colonic carcinomas not only have
higher rates of MSI but also an overall reduced tendency to
chromosomal instability, with gross chromosomal alterations
presumably occurring as later genetic events compared to rectal
carcinomas. | de |
