The aim of this study was to show that controlled release of recombinant human bone morphogenetic protein 2 (rh-BMP-2) trans a slow release polylactic acid carrier can repair critical size defects in the rat mandible. Polylactic implants with two different concentrations of rh-BMP-2 (96 mg rh-BMP-2/ polylactic implant and 48 mg rh-BMP-2/ polylactic implant) were produced and implanted into 45 male kyoto-wistar rats. For comparison a collagen sponge with the same concentration groups of rh-BMP-2 was tested as another carrier. Unfilled defects in both groups served as empty controls. After 6, 13 and 26 weeks implants of each carrier and concentration group were retrieved from five animals each and prepared for histological evaluation. New bone formation was measured by computer assisted histomorphometry. In both carrier groups the higher rh-BMP-2 concentration induced more bone formation than the lower. This was significant for the polylactic implants after 6 and 26 weeks and for the collagen sponges after 13 weeks. Moreover a bridging of the critical size defects could be found. Both concentrations induced significantly more bone formation than the empty controls.

Ziel dieser Studie war es zu zeigen, dass
eine kontrollierte Freisetzung von rekombinant hergestelltem Bone
Morphogenetic Protein 2 (rh-BMP-2) aus Polylaktidimplantaten in der
Lage ist, Defekte kritische Größe im Unterkiefer von Ratten zu
heilen. Dazu wurden Polylaktidimplantate mit zwei verschiedenen
Konzentrationen an rh-BMP-2 (96 mg rh-BMP-2/ Polylaktidimplantat
und 48 mg rh-BMP-2/ Polylaktidimplantat) hergestellt und in die
Mandibula von 45 männlichen Kyoto-Wistar-Ratten implantiert. Zum
Vergleich wurde ein Kollagen mit den gleichen Konzentrationsgruppen
an rh-BMP-2 als zweites Trägermaterial getestet. Leerdefekte
dienten in beiden Carriergruppen als Kontrollen. Nach 6, 13 und 26
Wochen wurden Implantate von jeder Konzentrations- und
Trägermaterialgruppe von je 5 Tieren entnommen und zur
histologischen Beurteilung vorbereitet. Mit Hilfe eines speziellen
Computerprogramms wurde der neu entstandene Knochen
histomorphometrisch vermessen. In beiden Carriergruppen führte die
höhere rh-BMP-2 Konzentration zu mehr neu gebildetem Knochen als
die niedrigere. Dies war bei den Polylaktidimplantaten signifikant
nach 6 und 26 Wochen und bei den Kollagenträgern nach 13 Wochen.
Zudem konnte eine Defektüberbrückung beobachtet werden. Beide
Konzentrationen induzierten signifikant mehr Knochenneubildung als
die Leerkontrollen.
